The 56th Academic Conference of the Kanto Block, Japan Hospital Pharmacists Association (August 22, 2026)
Symposium 9
“In-Person Participation! How Do Adverse Event Reports Lead to Safety Measures? —Sharing ‘Uncertainties’ and Considering Information Useful for Evaluation—”
Adverse event reports begin with observations made at medical institutions and lead to safety measures through individual case evaluations, follow-up investigations, and aggregate analyses conducted by pharmaceutical companies and the PMDA.In this presentation, we provided an explanation of how adverse event reports are evaluated and utilized for safety measures—such as package insert revisions and safety alerts—while providing an overview of the entire safety measure cycle.
It was also explained that reports from healthcare professionals serve as the starting point for safety measures, and that it is important to share initial reports even at stages when information is incomplete. Additionally, the significance of follow-up investigations and how reported information is utilized in safety assessments were introduced.
Furthermore, using actual examples that led to package insert revisions, the presentation demonstrated that the accumulation of information from individual case reports and follow-up studies serves as one of the key foundations for safety measures. We emphasized that a single report has the potential to contribute to ensuring patient safety in the future, and discussed the importance of collaboration among healthcare professionals, pharmaceutical companies, and the PMDA to support patient safety.
We hope that the content of this presentation will help promote understanding of the significance of adverse event reporting and further advance the sharing of safety information among healthcare professionals, pharmaceutical companies, and the PMDA.
